Advantages of Erbium:YAG Laser Over Other Lasers for Laser Resurfacing

There are two types of resurfacing lasers in common use: Erbium:YAG and carbon dioxide, CO2. Although the general principles of skin resurfacing with these lasers are similar, there are significant differences between them. The Erbium:YAG laser is similar to the carbon dioxide laser in that it targets water in the skin. Because theses lasers target water and water is everywhere in the skin, the laser beams will virtually vaporize and remove skin layer by layer. The special properties of the Erbium:YAG laser are due to its wavelength in the infrared region, at 2940 nm. At this wavelength, water absorbs over ten times as much energy as it does at 10,600 nm, the wavelength of the carbon dioxide laser. Consequently, virtually all of the laser energy is consumed by water, heating it and vaporizing tissue. Thus, very little energy is left to scatter into the skin and produce nonspecific heating. This offers several advantages, including less pain, less risk for undesired color changes of the skin, 30-50% faster healing, and less redness of the skin after healing compared to CO2 laser resurfacing.

The Laser Resurfacing Procedure at Nu Vela

The state-of-the-art Erbium:YAG laser for laser resurfacing used at Nu Vela is the Contour TRL™, manufactures by Sciton technology. This is an ablative laser in the sense that skin resurfacing with this laser involves a short period (3-10 days) of healing or "social downtime" after the procedure.

The Contour TRL™ is unique because it offers state-of-the-art modalities in one system by treating a wide variety of skin conditions and giving excellent results. The special scanning device of Sciton's Contour TRL™ moves the laser beam across the skin in a predetermined pattern and gives your physician precise control to gently remove a uniform depth of damaged skin with each pulse or pass. Moreover, this device allows your physician to match the desired result with the desired amount of downtime through independent selection of the amount of thermal coagulation for collagen buildup; and the depth of laser peel, from very superficial (10 microns) to very deep (300 microns). When used for microlaser peels, the chosen range is between 10 and 60 microns.

Laser resusurfacing with the Contour TRL™ can be performed with topical numbing cream for more superficial treatments, while deeper and full face resurfacing is usually performed with nerve blocks and pain pills. Added safety and comfort are achieved by the highly effective super cold air, continuously provided by the Zimmer cooling machine. In most cases, only a single treatment is required. Redness following treatment usually lasts 2–3 weeks after surgery, during which new and fine skin, free of many wrinkles will emerge. Improvement in the appearance of the skin continues to occur for the next 8–12 months. You can start putting make-up in 3–10 days, depending upon the depth of resurfacing and your age. In the early weeks and months following treatment, your skin will be very susceptible to sun damage. Accordingly, you should always wear sunscreen and avoid prolonged exposure to the sun.

Sciton's ProFractional-XC Therapy™ is your solution for achieving excellent resurfacing results with minimal downtime. Unlike full field resurfacing which removes the entire surface area of a treatment site, this skin resurfacing technology delivers Erbium:Yag laser energy to create thousands of microscopic "wounds" in the skin which are surrounded by areas of healthy, untreated skin. The zones of untreated skin invigorate the body's natural healing process and allow your skin to heal in a fraction of the time because you are not treating the entire skin surface all at once; whereas the treated areas stimulate production of new collagen, eliminate pigmentation, plump up the skin and smooth out wrinkles, lines, scars and other irregularities.

What truly sets ProFractional-XC Therapy™ apart is TRL technology. For the first time, ablation of superficial skin layers with a choice of wound-coverage densities and thermal coagulation of deeper lasers can be independently selected. This allows for customization of the treatment, be it for skin contraction and tightening (due to thermal coagulation) or for resurfacing acne scars or fine lines and wrinkles.

ProFractional-XC Therapy™ can be used on all skin types, both on face and body. Most patients describe the treatment as having a "prickling" sensation. A topical anesthetic is available if desired. Patients receiving Profractional Laser treatment generally notice results immediately that progressively improve. One to four treatments, spaced 3 - 6 weeks apart, offer the effective results of the fully ablative laser resurfacing with virtually no pain and little downtime (2 to 3 days at a time). Your physician will personalize a treatment plan based on the level of improvement you desire. Following treatment it is extremely important to avoid direct sun exposure for approximately three months. Laser Resurfacing Vs IPL (Intense Pulsed Light)

While the laser beam in skin resurfacing lasers, whether CO2 laser or Erbium:Yag, targets the water content of the skin and vaporizes entire layers of skin cells, including pigmented and normal cells, IPL photofacial treatments are more color specific in their targeting and go after only pigmented spots and patches, leaving the surrounding skin minimally traumatized. As a consequence, Sun and age spots removal with IPL treatment is less painful and recovery is much shorter. However, IPL will not remove moderate to deep wrinkles or take care of all types of acne scarring. Note that the lower face in male patients should not be treated with IPL as the treatment may also target the dark facial hair, resulting in unintended permanent hair reduction. A superficial laser peel would be a better and safer alternative for these patients.
